Certificate Content
| 1. | Complete the following fields about the Health Certificate to be uploaded |
| • | State of Origin - Select the state of the physical location from which the animal(s) will originally depart to begin travel to the country of destination. This may be a farm, a pre-export isolation center, or a pet-owner's house. The state of origin must be a state/territory where the veterinarian is authorized to perform accredited work. |
| • | Accredited Veterinarian - The list will display all Accredited Veterinarians in the organization who are accredited in the State of Origin selected above. |
If the certificate was created by a veterinary clinic:
| • | The list will display all Accredited Veterinarians in the organization who are accredited in the same state as the Commodity’ State of Origin selected above. |
| • | Result: The Corresponding License and Accreditation Numbers display. |
| • | Note: The List of Accredited Veterinarians is populated based on the Commodity’s State of Origin that was selected. If no state was selected, then the AV drop down will display No AVs found for State Selected. |
| • | This is a required field. |
If the certificate was created by an exporter organization:
| • | Select either an Internal Accredited Veterinarian (choose this option if there is an AV in their organization) or External Accredited Veterinarian (choose this option to send the certificate to a Veterinary Clinic). |
| • | A list will display all applicable AV’s (if internal AV is selected) or veterinary clinic (if external AV is selected). |
| • | Estimated Date of Shipment |
RESULT: The Upload Filled PDF Health Certificate screen appears with the Tracking # and Certificate Content Commodity Type and Destination Country displayed at the top of the screen.
HELPFUL HINT: You may enter certificate information in any order by selecting the links at the left of the screen. In this Help file, data entry into the certificate will be explained sequentially as if using the Next button. At the bottom of each page the Previous, Save as Work in Progress, and Next buttons are available.
The type of Certificate (Certificate Content) and the unique Certificate Tracking Number is always displayed at the top of each Step by Step screen.
