Certificate Relationship

The Certificate Relationship Report provides information regarding re-issued health certificates including: issued date and current status of the health certificate entered as search criteria and any associated health certificates with their status and replacement reason.

To generate the Certificate Relationship report, complete the following steps:

1. Select the View Reports link from the left side of the screen.

    RESULT:  The View Reports page appears.

2. On the View Reports page, under Certificate Reports, select Certificate Relationship.

    RESULT:  The Record of Public Funds Received page appears.

3. Enter a Certificate Number.

4. Click the Generate Report button.

    RESULT:  The report can be Opened or Saved.
