Statement Data Mapping

The Statement Data Mapping Page allows you to define the certification statements needed for the certificate definition.

Statement Fields table displays the fields that have been retrieved from the statement fields annotated on the health certificate uploaded for this definition. Form fields for statements should be prefixed with 'ST' to ensure the AV entered data is saved correctly.

•	Enter the Field Labels to be displayed to the AV for data entry.

•	You can also enter Helper Text.

•	Select the Data type.

•	Enter the drop down values if any.

The Statement Reference File will be displayed at the bottom.

NOTE: If there are tables embedded in the certification statements, they are defined in the Certification Table Data Mapping screen.

HELPFUL HINT: At the bottom of each page the Previous, Cancel, Save, and Next buttons are available.

The type of Certificate Definition (Certificate Content) is always displayed at the top of each Definition screen.
