The Template Details page field descriptions are as follows:
To update a template, complete the following steps:
| 1. | Enter the details to be saved in this template. Select 'Save' to save the information and return to the list of templates.
Items marked * are required. |
Template Name
| • | A meaningful name for the certificate template. This will allow you to easily identify the template you wish to use for creating a new certificate. |
| • | This is a required field. |
Type of Admission
| • | Select Permanent Import, Re-entry, or Temporary Admission from the drop-down list. |
| • | This is an optional field. |
Commodity Origin Information
| • | Enter the Place of Origin Name, Street Address, City, State/Territory, Zip Code for the commodity's origin. |
| • | These are all optional fields. |
Consignor
| • | Select the consignor from the drop-down list. |
| • | This is an optional field. |
| • | To add a new consignor, click the Add button.
RESULT: The Consignor Details page appears where you can enter information to add a new consignor. |
Consignee
| • | Select the consignee from the drop-down list. |
| • | This is an optional field. |
| • | To add a new consignee, click the Add button.
RESULT: The Consignee Details page appears where you can enter information to add a new consignor. |
Port of Embarkation/Border Crossing
| • | Select the name of the port of embarkation/border crossing from the drop-down list. |
| • | If the Port being used is not shown in the drop-down list, then select “Other” and enter the Details in the field labeled “If Other, More Details." The Details are required if “Other” is chosen. |
| • | This is an optional field. |
Means of Transport
| • | Select either Airplane, Ship, Train, or Truck as the means of transport from the drop-down list. |
| • | This is an optional field. |
Commodity Table Design
| • | If the template will be used for User-Defined Health Certificates, you can select a commodity table design. If a commodity table design is selected in a template used for a certificate that has a VEHCS predefined commodity table, the selected commodity table design will not be used. |
| 2. | Click the Save button to save the information and return to the list of templates. |
